  • Submit this Certification as soon as possible, after mitigation construction is complete or as soon as it is known changes are needed, to allow time for Environmental Section II to review and make any necessary field changes before the contract is terminated. The form can include multiple sites.

  • 1. Did the permanent impacts to wetlands or other waters identified in the plans which required mitigation happen to the extent shown in the plans?
  • The wetland impact table in the plans shows the designed impact acreage and if mitigation was required based on final design.

    If the impact areas changed, attach documentation (i.e. brief description of why the impact changed; table with the wetland number(s); and note designated impact acreage, actual impact acreage; in the as-builts.

  • 2. Is the location of the mitigation the same as the location shown in the plans?
  • If the location is to be moved, contact Environmental Section II as soon as possible to verify location. A field visit with the Environmental Section Staff will most likely be needed to verify and help answer the questions below.

  • 3. Do the elevations of the onsite mitigation area match the plans?

  • 7. Submit “As-Builts” (pdf) surveyed after topsoil placement of the onsite mitigation area(s) and photos (jpg) of the site as an attachment to this certification. Include permanent impact areas if they have changed.
