Please complete this form each month for all infants and toddlers who are not receiving CCAP. Enter the information exactly as it appears in your licensing records and enrollment files. Payments are processed on the 10th of every month. All payment requests need to be submitted by the 5th of the month to be processed in that month. If submitted after the 5th, all eligible payments will be processed in the following month.
